In addition to an individual consultation, I have included a form in a checklist format so that certain questions and issues can be defined. Often clients prefer to fill out such forms in addition to speaking with the designer directly over the phone or at the property.

Please do not take this form as a comprehensive set of criteria. Each individual client's needs and issues are unique. It is only used to supplement a more thorough discussion that flushes out things that cannot be stated in a checklist format.
